1&2) Look in the tutorial section and look under the tutorial Importing Skins.
3) A skin should be a BLP file (BLizzard Image MaP).
To convert a skin from JPEG, BMP, or TGA to BLP, you can use Warcraft 3 Viewer, available for download in the Tools section of this site.
