Way of crating icons?

Way of crating icons?

I want to know, how can you create so beautiful icons? Are drawing them on list of paper, then you scan this and edit icons in a program, or you just create icon direct in let say Photoshop?
Wich program do you use for drawing icons?

I use photoshop to create my icons. I doubt that anyone paints them on an paper and scan them.

I use photo shop only.

Thisis because of the vast variety of tiny little effects, and a litle of good usage of dodge and burn as well, and a couple neat little paint filters to do some effects
